This membership agreement, when signed by the applicant and accepted by Cellular Health and Beauty (Evangelia and/or Kelly Vensel), constitutes a binding contract between Cellular Health and Beauty (a private membership organization hereinafter referred as to the ‘Association’) and the individual executing the signature page of this Agreement.  Membership rights, privileges and obligations commence when the Agreement is signed by both parties as of _______________________(date). With the signing of this agreement, the acceptance by the Association, and payment of a non-refundable fee of $10.00 USD,  I accept the offer made to become a member and I express my agreement with the following DECLARATION and MEMORANDUM OF UNDERSTANDING and agree as well to be bound by its terms.

  1. This association of members hereby declare that our primary purpose is to protect and maintain our right to freedom of choice regarding alternative therapies, alternative modalities of treatment, health care decisions and the health improvement practices that we choose to receive by asserting our constitutional, contractual, and civil rights.

  2. As members, we affirm our belief that the Constitution of the United States guarantees all Americans, particularly members of the Association, the right of freedom of association, speech, assembly, belief and associated activities. These are our inalienable rights.

  3. We declare and assert the right to select from our membership those who can be expected to give the wisest counsel and advice regarding alternative therapies, alternative modalities of treatment, health care decisions and the health improvement practices and to authorize those members who are most skilled to facilitate the actual performance and delivery of health assistance and improvement methods that they and we deem appropriate. We assert these rights under the Federal and State constitutions.

  4. We claim our freedom to choose and accept for ourselves the types of health care modalities that we think are best for determining the cause and correction of our health challenges. We do this in order that we might achieve optimal health and well-being. We reserve the right to include traditional, non-traditional or even unconventional health care options whether or not they have been approved or accepted by any entity.

  5. All business, communication and practices conducted between Association founders and members is understood to be in the private domain under all protections acknowledged and guaranteed by the Constitution of the United States of America.

  6. We believe that the Constitution guarantees our members the rights of free speech and the rights to be free from unreasonable search and seizure, the right to not incriminate ourselves, and the right to freely exercise all other unalienable rights as granted by God and guaranteed by those Constitutions, Charter, and Statutes.

  7. Membership to this Association may be terminated by An Association Founder at any time.  I also can terminate my membership at any time by providing written notice to the Association.

  8. I understand that, since The Association is protected by the First and Fourteenth Amendments to the U.S. Constitution, it is outside the jurisdiction and authority of Federal and State Agencies and Authorities concerning all complaints or grievances against The Association members or other staff persons. All rights of complaints or grievances will be settled by an Association founder and will be waived by the member for the benefit of The Association and its members. By agreeing to this membership form I agree that I have sought sufficient education to determine that this is the course of action I want to take for myself.

  9. I enter this association under my own free will and all activities are deemed a private contractual matter.  I will not share any activities or information with any local, state, federal official(s), investigative or otherwise.  I fully agree not to pursue any course of legal action against a fellow member or founder of The Association, unless that member has exposed me to a clear and present danger of substantive evil, and upon the recommendation and approval of the Association.

  10. The member acknowledges that membership does not give the Member an equity or ownership interest in the Association, nor does it give the Member a vested or prescriptive right or easement to use the Facilities.

I, __________________________________, hereby apply for membership in the Association and express my agreement with all information, declarations and memorandum of understanding presented above.
